We request that patients give 24hr notice for non-emergent cancellations and 12hr notice for emergency request cancellations. Exceptions can be given depending on the nature of the reasons or emergency for cancellation.
We have a call in cancellation list for patients who do not show for 2 or more appointments with no communication. Cancellation within 1 hour of appointment time will be treated as a no show.
If a patient fails to show for 3 consecutive appointments with no communication or communications does not fall into any of the policy outlines above then patient will be placed on a do not appoint list and will be subject to removal from the practice after approval from Dr. Swenson.
A letter will be sent via mail and email with explanation of removal and copy of our cancellation policy.
